The international spread of infectious agents that have developed resistance to antimicrobials must be regarded as a global problem requiring common strategies, especially in developed countries. Some key elements of an overall strategy for the containment of the spread of resistant bacteria of animal origin are education and prudent use of antibiotics among veterinary practitioners and food animal producers, research, monitoring of resistance and monitoring of the use of antimicrobials in animal production.

The overall objectives of a surveillance programme concerning antibiotic resistance in bacteria of animal origin are summarized in the final recommendations of ARBAO, Concerted Action FAIR PL 97-3654, which stipulate that it is crucial to collect accurate and reliable data of resistance and antibiotic consumption in food producing animals and companion animals.

The surveillance of antibiotic resistance should target different bacterial species such as bacteria exclusively causing clinical infections in animals (“veterinary” pathogens), bacteria isolated from animals but capable of also causing human infections (zoonotic bacteria) and commensal bacteria isolated from healthy animals (indicator bacteria).

Two different concerted actions (EU IV° and V° Framework Programme) have been conducted to create a network of national veterinary reference laboratories in Europe and to establish a surveillance system for monitoring the occurrence and the emergence of antibiotic resistance in bacteria of animal origin (ARBAO and ARBAO II). In Italy, the National Veterinary Reference Centre for Antibiotic Resistance (IZS Lazio e Toscana, Rome, Italy), is an active member of the EU network. The backbone of the monitoring system is based on collecting representative data from different regions (northern, central, and southern Italy) and from food animals (bovine, ovine, swine, and poultry) and companion animals. The information on the resistance situation is provided for the three categories of bacteria of animal origin: animal pathogens (i.e. Pasteurellaceae, Actinobacillus pleuropneumoniae, Staphylococcus aureus, Streptococci), zoonotic bacteria (Salmonella, Escherichia coli EHEC), and indicator bacteria (E. coli, Enterococci). The first report of the Italian monitoring system on antibiotic resistance in veterinary medicine (2003) was issued at the end of 2004. From 2004 onwards, the surveillance system has also included the monitoring of resistance among the zoonotic thermo-tolerant Campylobacter species from food animals (poultry, swine, bovine). Representative examples from the Italian veterinary monitoring system (2003) are discussed.
