The aim of this study was to evaluate the distribution of drug efflux system genes from RND family (adeABC, adeDE, adeIJK, adeFGH and adeXYZ), MFS family (AmvA), MATE family (abeM) and SMR family (abeS) and its association with phenotypes of resistance to quaternary ammonium compounds (QACs) in clinical isolates of Acinetobacter baumannii growing in biofilm and planktonic forms. To evaluate the resistance phenotypes we used tests recommended by the Association of Official Analytical Chemistry (AOAC) and a commercial disinfecting agent which contains 10% v/v QAC. Two assays for biofilm formation -colony biofilm and microtiter plate biofilm- were used in the study. We analyzed 50 strains of A. baumannii isolated from patients and inanimate surfaces from environment of Intensive Care Unit (ICU) and Neonatal Service (NS) from the Caracas University Hospital. When cells grow in planktonic state 26 of 50 strains exhibited resistance phenotype towards the QAC disinfectant agent. 85% of this isolates were positive for PCR amplification of adeIJK genes, representing this operon the most prevalent among tested strains, followed by adeABC genes (70%) and adeFGH genes (65%). In 84% of strains tested, more than 5 efflux pumps genes were found. These results suggest that multiresistant bacterial strains persist in inanimate surfaces of the hospital services, acting as genetic reservoirs for multiple resistance genes. Beside, all bacteria tested when growing in biofilm were able to resist the action of QAC disinfectant agent, even during exposure times longer than one hour. Bacteria cells into a biofilm are more resistant to this disinfecting agent than its planktonic counterparts.
