Best infant and young child feeding practices are essential for optimal nutritional status, growth, development, health and basic survival, especially in Africa where the majority of the world’s child under-nutrition occurs. Reducing child mortality by 2015 is one of the eight Millennium Development Goals. To achieve this goal, the World Health Organisation encourages exclusive breastfeeding for the first six months of life, followed by supplemented breastfeeding for two years and beyond with the introduction of nutritionally adequate and safe complementary foods at six months. Although breastfeeding initiation is common in Africa, exclusive breast feeding is only practiced for a short time. The World Health Organisation recommends formula feeding only within acceptable, feasible, affordable, sustainable and safe circumstances. Poor communities, such as those in Africa, rarely meet these conditions and exclusive breastfeeding has shown higher survival rates, even in communities with a high burden of Human Immunodeficiency Virus infection and Acquired Immunodeficiency Syndrome. There are many reasons why mothers do not breast feed, including the perception of insufficient milk, inadequate education and support, social and family influences, separation from infants and local beliefs. Reasons for using favouring formula also include the importance of controlled feeds and shared responsibility, family and community pressure and the mother’s health status. Within this scenario optimal formula feeding is a challenge because of affordability, sustainability, sanitation and low literacy.
