Plasma glucose concentration after ingestion of carbohydrates can be critical for consumers with metabolic diseases such as obesity and diabetes. These consumers demand foods with low glycemic response and glycemic index, which is an important issue in marketing of foods. So far, these indexes are mostly measured in humans. Human trials are expensive and time consuming. A rapid, reliable and cost-effective method to predict carbohydrate glycemic response is necessary. We developed an in vitro method to measure the digestibility of carbohydrates during transit through the stomach and small intestine and the availability for absorption of monosaccharides, and to predict glycemic response. The technology (patent pending) is named TIM-Carbo and comprises the following parts: (1) a computer-controlled dynamic model of the upper gastrointestinal tract that closely simulates the digestion of carbohydrates by salivary and pancreatic enzymes and their further degradation to monosaccharides by brush border enzymes; (2) a rapid analysis of total available glucose and fructose, and (3) a kinetic in silico model to translate analysis results to predicted glycemic response. We validated the TIM-Carbo technology by using 21 different carbohydrate containing food products. Predicted glycemic response curves obtained with TIM-Carbo were compared with those from in vivo data of the same products. All predicted glycemic response curves deviated less than 10% from the in vivo data and an accurate prediction of the C_max of plasma glucose was established. In conclusion, glycemic response was reliably predicted with the TIM-Carbo technology, as a rapid and cost-effective in vitro tool.
