Wheat bran is the only cereal fibre source recognised by the US FDA Laxative Monograph as a source of fibre that promotes regularity (21 CFR 334.10 and 334.52 (d)(2)). Similarly, Health Canada has recognised the superiority and efficacy of wheat bran for claims on ‘promotion of laxation or regularity’ for foods, when a reasonable daily intake of the food contains a minimum of 7 grammes of dietary fibre from coarse wheat bran. The purpose of this review was to determine the effect of wheat bran consumption on various measures of bowel function and regularity, including stool weight (i.e., faecal bulk), stool frequency and intestinal transit time. Results of the present review identified 54 clinical trials that showed varying levels of bowel functional benefit associated with wheat bran consumption in adults and/or children. Of these trials, 35 studies provide substantiation that wheat bran intake increases faecal bulk, 17 studies provide support that wheat bran intake produces a greater frequency of bowel movements and/or number of defecations, 20 studies report that wheat bran consumption increased intestinal transit time, and 7 studies demonstrated that wheat bran intake decreased laxative use. Of the studies that considered only wheat bran as the active treatment (i.e., not a combination of wheat bran plus another agent), the wheat bran dosage range was approximately 10 g to 72 g per day, while the majority of studies had a range of 20 to 30 g of wheat bran daily. While several studies discussed within this report suggest that daily wheat bran consumption does not significantly affect stool weight and/or transit time, a critical assessment revealed several weaknesses inherent in these articles, making it difficult to draw scientific conclusions from this subgroup of studies.
