Several epidemiological studies reported an inverse relationship between plasma high-density lipoprotein (HDL) cholesterol levels and atherosclerotic cardiovascular disease. However, therapeutic interventions targeted at raising HDL-cholesterol failed to improve cardiovascular outcomes, suggesting that HDL components distinct from cholesterol may account for the anti-atherothrombotic effects attributed to this lipoprotein. Sphingosine-1-phosphate (S1P) – a lysosphingolipid regulating a variety of physiological and pathological responses via S1P receptors (S1P_1-5) in a variety of different organ systems, including the cardiovascular system – has been identified as an integral constituent of HDL particles. This chapter focuses on evidence in support of the concept that S1P-rich HDL play a role in different HDL atheroprotective properties and may represent a potential target for therapeutic interventions.
