Chronic obstructive pulmonary disease (COPD) is a leading cause of morbidity worldwide. Research has shown that patients with COPD and other non-malignant respiratory diseases (NMRD) in the United Kingdom and Canada may have less access to end-of-life care services than patients with other respiratory diseases such as lung cancer. Discussing preferences for end-of-life care can be difficult for patients, carers and health professionals, however it is essential to ensure that the patient’s wishes are met, particularly when resources are scarce. This study will compare end-of-life communication in NMRD in Canada and the United Kingdom. Specifically, how health professionals know a patient is ready and willing to discuss their end-of-life care preferences. Patient and carers experiences of such discussions will also be examined. Methods will include a comparative systematic review, the Delphi method and individual interviews to develop an understanding of end-of-life discussions with people with NMRD. From the results we hope to determine what each country can learn from the other to optimise end-of-life discussions. A tool will be developed to aid health professionals apply the findings of the study to their practice by increasing awareness of possible factors affecting end-of-life care discussions in NMRD.
