Abstract

The African continent is highly populous and multilingual with diverse, rich cultures and societies; commensurately, the burden of communicable and non-communicable diseases on the continent is quite high. Within this complex, multifaceted African medical kaleidoscope, researchers are faced with a myriad of ethical challenges. For researchers intending to use dying African patients as human subjects for biomedical or behavioural research, there must be a high level of sensitivity and malleability with regards to the application of research ethics principles, with the researchers adopting a culturally sensitive approach to produce a viable and relatable research outcome, especially as several issues about death and dying are considered sensitive and taboo subjects in many African communities. Researchers must be aware that dying patients are a special type of vulnerable population, interaction with which is liable to produce unanticipated, and perhaps novel ethical dilemmas. This chapter explores challenges faced when researching on dying African patients and ethical issues researchers face when conducting research at the end-of-life in this subset of patients. It also elaborates on the principles of Afro-cultural research ethics as an ethics in-context, and it discusses how medical researchers can adopt the culturally responsive relational reflexive ethics (CRRRE) principle from the social sciences to ensure that research conducted on dying subjects are culturally relevant and that they provide an ethically acceptable body of evidence for African medical research and global community.
